Cost: 18 and younger, insurance will be billed. For those without insurance or underinsured there will be no cost. Teachers and Staff, can bill insurance or cost is $20
If you would like to have your child receive the flu vaccine a consent form will need to be filled out and returned to school. You can download the consent form here in English or Spanish.
